Supported Housing/ Domiciliary Care
Ebony Health Care Services Ltd provides a flexible and adaptable accommodation and support service to adults and adolescents of 18+ at all levels, in both shared and self-contained units.
Support packages are tailored to suit individual needs, as agreed between the placing authority and Ebony Health Care Ltd and are regularly monitored.
As part of our commitment to well-being and fitness for all, all tenants will have access to gym facilities in Hornchurch as part of their tenancy in addition to staff support if required to attend gym sessions.
Ebony Health Care Ltd provides a CQC Registered Domiciliary Care service to a whole range of service users within the community. The service provides support to individuals on a practical and personal care level, which enables them to maintain their own independence and lifestyle.
The service may vary from a very small amount of support weekly e.g. provision of shopping support weekly to large and detailed packages of care providing several visits per day, this of course depends on the individual needs of the service user.
Services are available 24 hours a day and can be provided by our comprehensively trained and qualified personal support assistants, health care assistants, registered nurses, or sometimes a combination of staff grades.
We offer services primarily to people with mental health problems and individuals with learning difficulties.
Staff are experienced in all aspects of personal care requirements and are happy to assist with domestic duties, meal preparation and other daily living tasks such as:
- Shopping
- Socialising
- Employment and training
- Budgeting and rent payment
- Attending GPs and hospital
- Laundry
- Cleaning
- Mobility
- Chronic health management
Support with prescribed medication, orally and topically. Medication should be blister packed, or pre-prepared in ‘unit doses’ by a relevant other e.g. Chemist, district nurse, GP or a family member. Staffs are unable to administer medication from bottles from which they need to achieve a dose.
