Our care services can be accessed via social services or the community mental health teams or on a private basis. Following the initial referral, we will arrange to meet with the service user.
Care will be delivered by a Qualified Nurse (RMN) supported by a team of support Workers all qualified to a minimum of NVQ2. There will be group clinical supervision in addition to individual clinical supervision provided by the Clinical Supervisor. Call for more information.
Support Plans For Our Service Users
Support plans will be developed jointly with the service user and their needs and requirements will be agreed and a plan of care developed which includes details of our visit time to the service user and what tasks will be carried out during our visits. During this visit to discuss the service user’s needs, we also carry out risk assessments to ensure the health and safety of service users and staff.
We will review the care package initially at six weeks, or more frequently if needed, to ensure we are meeting the needs of the service user as per the plan of care.
Administration
We know that clear documentation and unambiguous information is vital. That is why the information in the residents home will always be correct, comprehensive and up to date.
Flexibility
We will do our best to provide the service which our commissioning colleagues require. We will also respond quickly to referrals and move in dates.
Safeguarding Adults
We value our clients and place a premium on their protection from any form of abuse. All our staff will be checked by the Criminal Records Bureau enhanced disclosure. All staff will be interviewed and references checked for authenticity before commencing employment. We will recheck all our staff under SOVA annually.
No Charge for Add-on Service
As part of our commitment to high quality care, we are pleased to be invited to reviews and meetings with social care professionals as well as meetings for service users forums.
Physical Health and Well-being
The health and well-being of our clients is of paramount importance to us. We will work with the GP practice to maximise the opportunities for the well-being of our clients through promoting gym membership and other healthy lifestyles.
There will be a daily provision of fruit and water in the communal flat for all clients to share.
We will have a physical Health Co-ordinator as part of the staff establishment in charge of carrying out physical health assessments, MUST Tool, arranging support for gym sessions, and arranging annual health checks for all clients.
This will be audited quarterly and reported to all concerned placing authorities, GP, and Community Mental Health Teams.
Assisted Living
Independent Living is about disabled people having the choice and control over the support they need to go about their daily lives.
We are committed to using technology to enhance the ability of our clients to live as independently as possible as they chose.
Individual Budgets and Direct Payments
All our staff will be fully trained in the implementation of individualises budgets and direct payments. Staff will be able to offer support with self- assessments questionnaires, support plans and liaising with the appropriate professionals.
Social Inclusion
We offer opportunities for all tenants to experience work, vocational training or college training.
We also offer opportunities for cultural and religious engagement through outings, local cultural events, and structured groups.
